*Essential Duties and Responsibilities:*
* Observe basic physical, functional, behavioral and / or psychosocial conditions, and analyze medical history;
* Assess and document the health status needs of individuals supported by Citadel Community Health Services;
* Assists with the facilitation and implementation of health care objectives of the Person-Centered Plan, collaborates with members of the Interdisciplinary Team (IDT) and other health care professionals in the community.
* Assess the individual based on observable information and information provided by Interdisciplinary Team;
* Review and assess the individual’s, and / or the significant support team system’s understanding of individual’s medical needs, proper preventative measures and proper treatment;
* Identify individuals as being medically frail or compromised using HRST assessments level 3 and above;
* Communicate with agency personnel, service providers, support coordinator, regional TA team and other interested parties to coordinate efforts, resolve operational problems, and improve the quality of individual care;
* May be required to provide technical support, teaching and training to non-licensed staff regarding health care services and medication administration
* If applicable, provide transportation of individuals following transportation policies and procedures.
* Complies with all applicable regulatory agency regulations and procedures as well as the policies and procedures of Citadel Community Health Services, Inc.
* Performs other duties as assigned.
*Education and Experience: *Must have active practical nursing licensure (Georgia); One year of work experience in ambulatory care setting preferred.
*Other: *21 years or older, Maintains valid driver’s license and insurable under their auto insurance.
Ability to pass criminal/registry background checks.
Clear tuberculosis screening
*Skills Knowledge & Abilities:*
* Working knowledge of developmental disabilities
* Knowledgeable in the State of Georgia Nurse Practice Act.
* Effective oral and written communication skills
* Knowledge of proper Medication Administration techniques and basic pharmacology.
* Working knowledge of computers to allow employee to access computer systems and applications to complete documentation, read and respond to email, and complete required on-line training.
* Ability to work as a team member, maintaining effective inter and intra-agency working relationships
* Ability to achieve and maintain certifications necessary for the job (including, but not limited to, First Aid, CPR, MANDT, Medication Administration).
* Skilled in emergency medical procedures.
Skilled in performing nursing assessments and emergency medical procedures.
