*JOB SUMMARY:*
Under the direct supervision of the Unit Coordinator, the Licensed Practical Nurse is responsible for assuring the provision of direct care of residents requiring assistance or supervision in personal, bathing, grooming, dressing and administration of medications.
The LPN shall provide general observation and health supervisions of the residents noting any changes in health and physical abilities, noting the need for medical attention or nursing services.
The LPN shall keep the Unit Coordinator/Administrator informed of changes in the resident conditions.
May inform resident condition changes to family/sponsor and physician according to the wishes of the Unit Coordinator or Administrator.
*QUALIFICATIONS: *
Must have genuine interest in Senior Adult care.
Previous experience in an assisted living environment is highly desirable.
Previous experience in supervision of direct care givers is desirable.
Enthusiastic, self-starter, and capable of using good judgment in making decisions.
Must be able to communicate well with staff, residents, sponsors, and physicians.
*EDUCATION:*
Completion of an accredited nursing school.
Licensed Practical Nurse with current Alabama license.
Experience in administration of medications.
Experience in working with persons with dementia is highly desirable.
*PHYSICAL DEMANDS:*
Standing, walking, bending, stooping, pushing and pulling.
Must be able to assist residents with ambulation and assist residents with transfers.
*WORK PERFORMED:*
* As the licensed nurse, will take responsibility for leadership of all functions of the facility in the absence of the Administrator or Unit Coordinator.
* Assign resident care/other job duties per facility policy.
* Administers ordered medications and treatments accurately, charts appropriately.
* Record medication changes in each resident's medical record and eMAR.
* Assist with receiving, checking in, and conducting match back verification on medication batches and eMAR.
* Make clinical rounds periodically during shifts.
Reviews uncompleted shift assignments to determine resident services needed or not completed, for appropriate interventions.
* Conducts shift report.
* Reviews individualized service plans for each resident based on level of care or changes in condition.
* Arranges consultations as ordered - Geropsychiatrist, podiatrist, dentist, home health, rehabilitation, therapy, etc.
* Coordinates physician and home health visits, ensuring required documentation is obtained.
* Consult with the resident's physician as needed.
Notify unit coordinator/physician for changes in resident conditions.
* Perform small dressing changes as needed.
* Assist with required resident monitoring and documentation as needed (BP, pulse, weight, etc.
)
* Notify the Unit Coordinator of significant changes.
* Assess Residents as needed related to falls, injuries, accidents or changes in overall health and wellbeing.
Notify Unit Coordinator/Administrator as appropriate.
* Complete incident reports and obtain witness statements for all falls, injuries or unusual occurrences.
* Work with the Resident Assistant in providing personal care services to the resident (bathing, dressing, grooming, etc.
)
* Work with Resident Assistants in performing housekeeping, laundry and assigned duties, (vacuuming, empty trash, dusting).
* Implements and participates in activities and wellness programs as needed.
* Monitors direct care staff performance to identify any significant gaps in the employee’s ability to perform/function competently.
* Provides a comfortable home-like atmosphere, keeping noise level appropriate, and monitoring staff actions and interactions, and ensures organization, upkeep and cleanliness of the unit.
* Maintains a safe environment for residents by observing the resident unit and entire facility with attention to monitoring life safety concerns such as system security, tripping hazards, smoking restrictions, etc.
* Maintains professional conduct at all times that reflects the community's philosophy of quality care and service delivery
* Keeps unit inventory of supplies at par level notifying Unit Coordinator of any needs.
* Assist with preparing, serving, and assisting/observing residents during mealtimes.
