* Qualifications:
* Successfully completed one of the following: o A training program including classroom and supervised practical training combined must total at least 75 hours, 16 of the 75 hours must be supervised practical training.
o A competency evaluation program that meets the requirements, referencing evaluation of aide’s performance of the task with patient.
o A nurse aide training and competency evaluation program approved by the State and is good standing on the State nurse aide registry.
* Has not had a lapse of 24 consecutive months where the aide has not provided services for compensation.
* CHPN: Willing to take steps to become certified as related to hospice and palliative care as certification becomes available.
* Physically and mentally capable of providing care to the hospice patient in the home care setting, and present a calm manner when in patient’s home.
* Understand and be comfortable with hospice philosophy of palliative care issues of death and dying, demonstrate empathy needs of the patient and patient family.
* General computer knowledge.
* Ability to perform with little or no supervision
* Good communication skills, being able to read, write and verbally report clinical information.
* Good observation skills, reporting, and documentation of patient status and the care of service furnished.
* Good interpersonal skills.
* Be able to multi task.
* Be able to handle emergency or crisis situation, with the support of the supervisor and other team members.
* Must meet physical requirements as specified (see Physical Requirements for RN, HCA).
* Orientation of Home-Care or Inpatient facility to be completed prior to working in the appropriate clinical area.
Core Guidelines:
* To be knowledgeable of and follow guidelines set forth in the Personnel Handbook.
* Share responsibilities as they become necessary (on-call, covering in the absence of fellow co-workers, assignments in Shepherd’s Cove, etc.
).
* Maintain CPR certification.
* Evidence of 12 hours of continuing education.
For HCA’s, in-service training must be supervised by registered nurse.
HMC offers education opportunities throughout the year.
However, the team member recognizes their responsibility in obtaining required education hours.
* Hospice of Marshall County requires annual completion of educational activities as defined in personnel policies and reflected in the personnel handbook.
* Hospice of Marshall County requires attendance at selected HMC support events and voluntary attendance at other HMC support events which are typically conducted outside the normal work day as defined in personnel policies and reflected in the personnel handbook.
* Attendance at weekly staff meetings or evidence of having read recorded staff meeting minutes.
* Evidence of current automobile insurance and clean MVR.
* Record time accurately and in a timely manner set by the guidelines.
* Clean drug screens.
* Participate in positive agency representation.
* Acceptance of HMC’s Values.
Job Specific Guidelines (but not limited to):
* Primary RN will provide direction and routine supervision along with ADON of services performed by the HCA in the home care setting.
* On-Call rotation as required.
* Follow basic infection control procedures.
* Perform housekeeping tasks in order to maintain clean, safe and healthy environment.
* Respect for the patient, his and her privacy, and his or her property.
* Assist in ambulation or ROM exercises.
* Perform hands-on personal care and follow appropriate and safe techniques in performing personal hygiene and grooming tasks, including items on the following basic checklist:
* Bath
* Sponge, tub, and shower bath
* Hair shampoo (sink, tub, and bed)
* Nail and skin care
* Oral hygiene
* Toileting and elimination
* Light housekeeping including light meal preparation
* Bed linen change
* Follow safe transfer and ambulation techniques.
* Follow normal range of motion and positioning.
* Knowledge of basic nutrition and fluid intake.
* Knowledge of proper body mechanics
* Maintain documentation that demonstrates that the requirements of standards are met.
* Must report changes in patient’s physical, psychosocial and spiritual needs to a registered nurse, as the changes relate to the plan of care and quality assessment and improvement activities.
* Must complete appropriate records in compliance with the agency’s policies and procedures.
