Qualifications: * A graduate with at least an Associate’s degree from an accredited school of professional nursing, with at least 1 year of experience (preferred) in home care or an acute care setting.
* Licensed by the Alabama Board of Nursing.
* CHPN: Willing to take steps to become certified as related to hospice and palliative care as certification becomes available.
* Competent in basic nursing procedures, e.
g.
, patient assessment of various age-groups, drug calculations and administration, nutrition and Standard Precautions of Infection Control.
* Able to understand and communicate well verbally and in writing with patients/families, physicians, and other staff members.
* Able to perform skills necessary for telephone and beeper use.
* Able to perform skills necessary for accurate reporting of time and mileage (see Itinerary).
* Motivated to work with the dying and bereaved and able to cope with continual multiple losses (patient deaths).
* Capable of adapting to and coping with sudden changes that may occur in the work situation.
* Ability to function in constant state of alertness.
* Comfortable with night-time calls, travel and home visits.
* Able to perform independently with little direction.
* General computer knowledge.
* Ability to perform with little or no supervision.
* Good communication skills.
* Good interpersonal skills.
* Must meet physical requirements as specified (see Physical Requirements for RN, LPN, and HCA).
Core Requirements: * To be knowledgeable of and follow guidelines set forth in the Personnel Handbook.
* Share responsibilities as they become necessary (on-call, covering in the absence of fellow co-workers, etc.
).
* Maintain CPR certification.
* Evidence of continuing education required by licensure.
HMC offers education opportunities throughout the year.
However, the team member recognizes their responsibility in obtaining required education hours.
* Hospice of Marshall County requires annual completion of educational activities as defined in personnel policies and reflected in the personnel handbook.
* Hospice of Marshall County requires attendance at selected HMC support events and voluntary attendance at other HMC support events which are typically conducted outside the normal work day as defined in personnel policies and reflected in the personnel handbook.
* Attendance at weekly staff meetings or evidence of having read recorded staff meeting minutes.
* Evidence of current automobile insurance and clean MVR.
* Record time accurately and in a timely manner set by the guidelines.
* Clean drug screens.
* Participate in positive agency representation.
* Acceptance of HMC’s Values.
Job Specific Guidelines (but not limited to): * Observe hospice philosophy of palliative care and professional standards of care in the home setting and professional environment in performance of duties.
* Assess and address patient/caregiver’s ongoing and changing needs for palliative (comfort) management of the terminal illness.
* Recognize patient/caregiver’s anticipatory or complicated grief or spiritual distress and provide moral and spiritual support.
* Make timely referrals for hospice services, e.
g.
, personal care, volunteer, chaplain, social work, or bereavement.
* Provide information to the patient/family about the disease process and treatment options, enabling them to make informed choices and participate in the hospice plan of care.
* Performs tasks within scope of practice as outlined by the Alabama Board of Nursing.
* Teach the patient and caregivers home nursing skills needed for the daily management of the terminal illness, including medication administration, personal and wound care, and dietary adjustments to meet the changing needs of the patient.
* Develop and follow the plan of care according to patient/caregiver needs and choices, physician’s orders, hospice philosophy of palliative care, and HMC policies.
* Act as a liaison between the patient/family and their physician, reporting to the physician significant changes in the patient’s condition.
* Communicate the plan of care to the Hospice Care Assistant (HCA), and LPN, provide routine on-site supervision of these team members.
* Report pertinent patient information to the on-call RN as needed.
* Maintain communications with other members of the HMC interdisciplinary team, participate in IDT meetings, and implement recommendation of the interdisciplinary team in the patient’s plan of care.
* Document activities in the patient record and maintain the record according to HMC policies and professional standards.
* Advise the nursing supervisor when called upon to perform tasks or assessments in which he/she does not possess competency.
* Seek out and take advantage of educational opportunities and training which support and increase hospice nurse competency in end-of-life and palliative care, securing the minimal number of continuous education units required for state licensure.
* Assist in the development of Nursing policies and procedures as directed.
* Comply with Performance Improvement (PI) reporting and/or data collection activities.
* Assist with chart auditing as directed.
Job Type: Full-time Pay: $24.
