Pay Rate: $11 to $12 per hour

Key Functions and Expected Performances for PCA positions
1) Provides individualized, plan of care, specific resident care under the supervision of a Licensed Nurse.
a.
Provides and completes basic hygiene according to facility standards.
b.
Maintains resident privacy and confidentiality during procedures and resident care activities.
c.
Turns and properly positions immobilized residents every 2 hours (or as required by plan of care) and perform skin inspections (as required by plan of care) with appropriate follow-up.
d.
Accurately obtains vital signs.
e.
Correctly documents vital signs.
f.
Reports abnormal findings in a timely manner (i.
e.
, significant changes in base line vital signs reported to nurse immediately)
g.
Monitors and documents intake and output (I & O’s), calorie consumption counts, and weights accurately according to unit guidelines.
h.
Communicates appropriate information to the ADL book and to nurse.
2) Performs selected procedures and treatments
a.
Follow facility specific policies and procedures.
b.
Provide privacy while performing all procedures and treatments.
c.
Provide resident/family education prior to performing procedures and treatments.
d.
Will attempt procedure or treatment no more than twice before soliciting advise from a more experienced care provide or licensed nurse/supervisor when needed to complete procedure or treatment.
e.
Will document according to facility policies.
f.
Communicates appropriate information to the charge nurse or Unit Coordinator, according to Nursing Standards.
3) Maintains employee / resident / family safety.
a.
Immediately notify the nurse supervisor of any resident leaving or missing from facility and initiate elopement procedure.
b.
Follow facility specific policy and procedure related to preventing and reporting suspected resident abuse.
c.
Follow facility specific policy and procedure related to employee safety practices, including reporting safety violations and operating equipment properly.
d.
Follows facility specific policies and procedures related to resident / family safety and confidentiality.
e.
Provides orientation of unit and surroundings to resident / family.
f.
Maintains resident / family confidentiality.
g.
Knowledgeable about use of equipment / supplies and documentation of usage.
h.
Documents appropriate safety measures per facility specific policies.
i.
Communicates appropriate information to the Charge Nurse or Unit Coordinator.
4) Participates in quality improvement activities
a.
Attends resident care plan meetings
b.
Attends staff meetings
c.
Attends in-services
d.
Communicate resident / family satisfaction information to the appropriate person
