*Bachelors Degree in mental health or social service related field.

Professional work involving the development and implementation of individual training programs.
Work includes assessing individual’s assets and deficits, instructing individuals in a group home setting and providing direct care for individuals.
May work evenings and/or weekends.
May be required to handle emergencies.
EXAMPLE OF DUTIES & RESPONSIBILITIES:
1.
Develops individualized plans with support and implements programming.
2.
May work evenings and/or weekends.
May be required to handle emergencies.
3.
Participates in administrative and clinical staffing.
4.
Responsible for timely entries in confidential files of individuals, as well as entries in
shift reports and behavior logs.
Responsible for monthly progress notes, social
histories and other administrative paperwork.
5.
Schedules and transports individuals to and from medical appointments.
6.
Responsible for scheduling individual’s activities.
7.
Responsible for direct care to case load of individuals providing daily living needs.
8.
Provides direct individual counseling.
9.
Responds to specific questions and requests from superior.
10.
Must complete and maintain MAC certification.
11.
Performs other reasonable, related duties as assigned by immediate supervisor and
other management, as required.
12.
Responsible for the storage and protection of individual records, including
compliance with the Heath Insurance Portability and Accountability Act of 1996.

Knowledge of behavior management and motivational programs and ability to
implement such programs.

individuals, other care givers and the community at large.
9 Working knowledge of Intellectual Disabilities or mental health problems, as well as
state requirements, modulations and assessment instruments, and the ability to
apply this knowledge in a clinical situation.
10.
Thorough knowledge of Board policies
MINIMUM QUALIFICATIONS:
Bachelors Degree in mental health or social service related field.
One year direct mental health/ Intellectual Disabilities experience.
Valid Alabama Driver's License.
Must be able to perform CPR (Cardiopulmonary Resuscitation).
The employee must be able to physically demonstrate that he or she is able to perform CPR including an on-the- ground demonstration of CPR proficiency"
