Chronic Care Management Coordinator (CCM)

Responsibilities: • Responsible for patient enrollment and consistent participation in program • Answering program enquiries • Assisting patients enroll using smart phones/tablets • Monitor program stock and supplies • Record keep enrollment and patient participation • Self motivated, ability to work independently and as part of a team • Evolve with project demands and enhance project as needed • Assume full responsibility of project needs at facility after training Skills: • Good verbal and written communication skills essential with a polite and cheerful disposition at all times • Candidate must be able to always multitask and possess a calm demeanor • Candidate MUST be competent/confident with word/excel and using technology such as smart phones and tablets • Prior promotion/sales experience preferred but not essential • Full training will be provided, previous experience at a medical facility is preferred Job Types: Full-time, Part-time Pay: $15.
